Feeling Tired on Mounjaro? Understanding Fatigue

Some Mounjaro patients experience fatigue, especially early in treatment. Learn why it happens and how to maintain your energy levels while losing weight.

Fatigue is a commonly reported experience among Mounjaro (tirzepatide) users, particularly during the first few weeks of treatment and after dose increases. While not everyone experiences it, understanding the causes can help you address it effectively.

Reduced Calorie Intake

The most straightforward explanation for fatigue on Mounjaro is that you're eating significantly less than before. When appetite decreases dramatically — as it often does with tirzepatide — your body receives fewer calories, which can result in reduced energy levels. This is especially true if your daily intake drops below approximately 1,200 calories.

To counteract this:

  • Ensure you're eating regular, balanced meals even if your appetite is reduced
  • Focus on nutrient-dense foods — lean protein, complex carbohydrates, healthy fats, and vegetables
  • Don't skip meals — consistent nutrition helps maintain stable energy throughout the day

Blood Sugar Changes

Mounjaro improves blood sugar control, which is beneficial, but if blood sugar drops lower than your body is accustomed to, you may feel fatigued or light-headed. This is more relevant for patients with type 2 diabetes, particularly those also taking other glucose-lowering medications such as insulin or sulphonylureas.

If you have diabetes, monitor your blood glucose regularly during the early weeks and discuss any patterns of low readings with your prescriber.

Dehydration

Gastrointestinal side effects such as nausea, vomiting, and diarrhoea can contribute to dehydration, which is a common and often overlooked cause of fatigue. Even mild dehydration can make you feel sluggish, reduce concentration, and cause headaches.

  • Aim for at least 1.5 to 2 litres of water daily
  • Increase your intake if you're experiencing GI side effects
  • Monitor the colour of your urine — it should be a pale straw colour

Nutritional Deficiencies

Rapid weight loss and reduced food intake can lead to deficiencies in key nutrients over time, particularly:

  • Iron — essential for oxygen transport; deficiency causes tiredness and breathlessness
  • Vitamin B12 — supports energy metabolism and nerve function
  • Vitamin D — low levels are already common in the UK due to limited sunlight
  • Protein — insufficient protein intake can contribute to muscle loss and fatigue

Consider asking your GP for blood tests to check these levels, especially if fatigue persists beyond the first month.

Sleep Quality

Some patients report changes in sleep quality while on Mounjaro — either difficulty falling asleep or lighter sleep than usual. Poor sleep naturally contributes to daytime fatigue. If this applies to you, practise good sleep hygiene: maintain consistent bed and wake times, avoid screens before bed, and keep your bedroom cool and dark.

When to Seek Medical Advice

Mild fatigue during the first few weeks is generally expected. However, see your GP if:

  • Fatigue is severe enough to interfere with daily activities or work
  • It persists beyond 4–6 weeks without improvement
  • It's accompanied by unexplained breathlessness, pale skin, or rapid heartbeat
  • You suspect your blood sugar is dropping too low

Your prescriber may recommend blood tests, adjust your titration schedule, or review your overall nutrition plan.
