In February 2026 the MHRA reported that its Criminal Enforcement Unit had seized roughly 19.5 million doses of erectile dysfunction medicines between 2021 and 2025, including 4.4 million in 2025 alone. Alongside the seizures it disrupted more than 1,500 websites and social media accounts selling medical products illegally.
No other medicine category in the UK is counterfeited at anything like this rate. There is a reason: ED is a condition people would rather not discuss, and embarrassment is the mechanism the illegal market runs on.
What Is Actually in Illegal Pills
The MHRA's account is blunt. Seized products have contained:
- No active ingredient at all
- The wrong dose — sometimes far too much
- Undeclared drugs
- Toxic contaminants
The dose problem is the dangerous one. Sildenafil and tadalafil lower blood pressure. That is fine at a controlled dose in someone screened for cardiovascular risk. It is not fine at an unknown dose in someone taking nitrates, and the combination can cause a catastrophic drop in blood pressure.
The Legitimate Routes
Over the counter
Viagra Connect (sildenafil 50mg) has been a Pharmacy medicine since its reclassification, and Cialis Together (tadalafil 10mg) followed. Both are sold from a pharmacy after a conversation with the pharmacist, who checks suitability and may refer you to a GP. No prescription, but not self-service either.
Prescription
Your GP, or a UK-registered online pharmacy with a prescriber. Generic sildenafil can be prescribed on the NHS for men with ED; other options are restricted to particular circumstances.
How to Check an Online Pharmacy
- Look it up on the register rather than trusting a logo. The GPhC register is public and searchable, and a legitimate site will name the pharmacy and its registration.
- Expect clinical questions. A service that sells without asking about heart conditions, blood pressure or nitrate use is not doing the one check that matters most.
- Expect a way to reach a prescriber. Two-way communication is what the regulator now expects of online supply.
- A UK address and a working phone number are a low bar that a surprising number of sites fail.
Red Flags
- No prescription and no pharmacist involvement for a prescription-only strength
- Prices dramatically below the market — the pharmacy has to buy the stock too
- Selling through social media, messaging apps or marketplace listings
- "Herbal" or "natural" products claiming Viagra-like effects, which have repeatedly been found to contain undeclared sildenafil
- Bulk quantities with no clinical assessment
The Cost Argument Does Not Hold
The usual reason people take the risk is price. But generic sildenafil is inexpensive from legitimate UK pharmacies, and the spread between the cheapest and dearest legitimate provider is often larger than people assume — which is exactly the gap a comparison is for. Shopping around within the legal market captures most of the saving without any of the risk.
If You Have Bought Something Suspect
Stop taking it. Report it to the MHRA through the Yellow Card scheme, keeping the packaging. If you have taken it and feel unwell — chest pain, fainting, vision changes, a persistent erection lasting more than four hours — seek medical help immediately and say what you took.
