Premature Ejaculation: UK Treatment Options

A common condition, frequently confused with erectile dysfunction, with its own treatments. What is licensed in the UK and what actually helps.

Premature ejaculation and erectile dysfunction are distinct conditions, though they often get bundled together by online "men's health" services. They can coexist — and one can cause the other, since anxiety about losing an erection can lead to rushing — but the treatments differ.

It is worth being clear which you have before buying anything, because ED medication does not treat premature ejaculation.

How It Is Defined

Clinically, it involves ejaculation occurring sooner than desired, with limited control over timing, and — importantly — distress or difficulty it causes. That last element matters: there is no universal correct duration, and the condition is defined partly by its impact rather than by a stopwatch.

It is broadly divided into lifelong premature ejaculation, present since first sexual experiences, and acquired, developing after a period of normal function. Acquired PE is more likely to have an identifiable underlying cause worth investigating — including erectile dysfunction, thyroid problems, prostatitis or a change in circumstances.

Treatments Licensed in the UK

Dapoxetine

A short-acting SSRI licensed specifically for premature ejaculation and taken on demand, one to three hours before sex. It is prescription-only. Side effects include nausea, dizziness and headache, and it can cause fainting, which is why it is started at the lower dose.

Topical anaesthetics

Lidocaine-prilocaine sprays and creams reduce sensitivity when applied before sex. Some products are available over the counter. The practical issue is transfer to a partner causing numbness, which is managed by timing and by washing off or using a condom.

Off-label SSRIs

Daily SSRIs such as sertraline or paroxetine are used off-label and can be effective, since delayed ejaculation is a well-known side effect of the class. This is a prescriber's decision and involves the usual considerations of taking a daily antidepressant.

What Also Works

Behavioural approaches have genuine evidence and cost nothing:

  • The stop-start technique and the squeeze technique
  • Pelvic floor muscle training, which has reasonable supporting evidence
  • Thicker condoms to reduce sensation

Psychosexual therapy is effective, particularly where anxiety is a driver, and is available on the NHS in many areas. For a condition defined partly by distress, addressing the distress is not a soft option.

Where ED Overlaps

If you have both, the usual advice is to treat the erectile dysfunction first. Some men rush because they are anxious about losing the erection, and resolving that removes the driver. Treating PE alone in that situation tends to disappoint.

Buying Safely

Dapoxetine is prescription-only, and the same rules apply as for any prescription medicine bought online: a UK-registered pharmacy, a real clinical assessment, and a way to contact a prescriber. Be particularly cautious of "delay" products sold outside the regulated market, which have repeatedly been found to contain undeclared active ingredients.

Worth Saying Out Loud

Premature ejaculation is common and treatable, and it is one of the conditions men are least likely to raise with a doctor. A GP or sexual health clinic will have discussed it many times. The alternative — self-treating from an unregulated source based on advertising — is both more expensive and less likely to work.
